- G6PD deficiency — methylene blue can cause haemolytic anaemia.
- Serotonergic medications, including antidepressants — methylene blue is a potent MAO-A inhibitor and the FDA has warned of serotonin syndrome in this combination.
- Pregnancy or breastfeeding.
